HOW TO SCHEDULE AN APPOINTMENT:

Please call 256-234-4443 to schedule an appointment.

CANCEL APPOINTMENTS:

Our office policy requires that you to cancel your appointment at least (2) hours prior to your appointment time.  

If you fail to arrive for your scheduled appointment you will be charged a fee.

WHAT TO BRING TO AN APPOINTMENT:

* Insurance Card

* Recent Immunization record

* A list of any medications currently being taken. You may also bring the medications with you.

*For your convenience, patient forms can be printed and filled out prior to your visit.
